I feel like it might be about drugs; "strange dichotomy" would be between the good, immediate feeling of getting high and how it sucks long term ("tomorrow I'll waste away"). Plus "insert the ink pen, jugular vein" is pretty explicitly referencing drugs. Maybe the whole thing is about a trip, heh.
