I think this song is about Matt useing drugs to escape his life and his problems..

Eventually he realizes that he can no longer use drugs as an excuse and he comes to terms that he needs to face his problems and come to a decision about what to do..

The last line probably means he can see what happening around him but is still unable to do anything about it, or rather that he doesnt want to change whats about to happen to his friends that ended up becoming back stabbers. He ends up crashing the car into the river and ever one but him dies.
He is happy with this and chants "stay a sleep" or stay dead in other words. He is so overwhelmed that there dead that he feels that "this is heaven" and is happy with the outcome.

'Her' is a reference to 'black' which may have been a nickname for a love interest of Matt's that probably the cause the whole back stabbing incident
