He fell in love with "the ghost" of the girl - he never saw the real person she was, just the person he wanted her to be and is ashamed that he didn't see how she really was from the beginning.
She ends up cheating on him, and lying to him..and when he finds out the truth he is crushed by it. She tells him it was just one big mistake and she regrets it. He takes her back as afterall she tells him her feelings haven't changed towards him -

he is just left wondering only why she did it and what she is trying to prove. It seems she is showing him that she could be with someone else if she wants to but he doesn't understand why she needs to do that to him.
She begs him back telling him "Don't say it's too late, 'Cause I need you to care" - and he takes her back as it is in fact him that needs her to care..
But he is beginning to see how she really is and that fact that what she does every time is "always place the blame, on something, you don't understand" and the fact that some things and some people never change - they're always going to blame other people or other things for their mistakes, and consequently keep hurting people and never taking responsibility or changing - "you, (will) Always be the same, 'cause some things, never change"

he knows that he should have seen that she wasn't worth it from the beginning and was bad for him, but he wanted it to work out with her so much that he let himself fall for her. It seems she had had bad past experiences and had been hurt and cheated on so many times before that she no longer could care enough or be faithful as she was too scared - from her "broken past" - but he thought he could fill the parts that were missing and show her that she could care and trust again but she kept returning to her old ways - he realised he should have seen it coming as "some things never change" - he is pretty much gutted that he realises he was wrong and he wasn't enough to change her. He was so focussed on how perfect he thought she was that her real self was hidden from his view - links to the whole "love is blind" thing..
His resentment towards her is shown by the repetition at the end -

But at the end of the day he couldn't change her, she was never going to treat him the way he deserved and this song is basically about how much it hurts to realise that...
