Dilaudid is an extremely potent, synthetic opiate. I have done dilaudid, as have many people I know. It is a dangerous, and addictive drug, very similar to heroin in a lot of ways. My interpretation of this song is that it is about a couple addicted to opiates.
The first two lines of the song really describe an opiate addiction quite well.
