These lyrics were intended to paint a picture of a literal battle scene in a place like Vietnam, and the ways a soldier might have of dealing with the horrors of it. This of course is the simplified interpretation, since everything I wrote on this album was an expression of my own "horrors" of everyday life. The images of war (being wounded, being chased by things trying to kill you, comforting comrades), are all expressions of how it sometimes feels for any of us in every day life. They guy you pass on the street might himself be dealing with a "monster" or "bleeding from the side" and needs a hand from a friend. He might be wanting to screem, but wonders if anyone will hear.
