To me, this song continues where "Dark That Follows" left off.
This girl is in a relationship with a guy, but she's having an affair with the singer on the side. She loves them both, and won't leave her boyfriend for the singer. At the end of the song, ("Dark that Follows") IMO, it seems that the singer gives up on her and stops waiting for her to leave her relationship for him.
This song starts out where I think she tries to leave her boyfriend, and they start fighting and she can't really leave him and keeps coming back to him. The first verse ("She brought her anger/she left her sympathy') seems to say that she goes to the singer to rant about the fights with her boyfriend, not realizing that it hurts the singer to hear about her boyfriend.
I think because of this aspect THEY start having their own fights (first chorus) about how she can't leave and he's thinking that because of this her own boyfriend might start to look better than him ("the promise").
The second verse is him apologizing for fighting with her, but thinks that loving her isn't enough, even though he's willing to keep trying.
Second chorus is another round of fighting, but this time her boyfriend is the threat to her "relationship" with the singer. The part that goes, "She's so dramatic, she says she's leaving... is that a promise, or a threat?" When she says she's leaving, he wonders if it's a promise (leaving her boyfriend) or a threat (leaving him).
Last chorus is again, more arguing, the singer wondering again if the boyfriend will be the one she chooses over him. Finally he realizes that she's never going to leave the boyfriend for him, that he is the promise she'll never keep, and the threat to her other, real relationhip.
