ok, here's my take:
i think this is one of those heart breaker songs where the guy did the hurting instead of the usual. it's almost an apology except he's actually asking her to be mad at him coz he deserves it. (Call me a name, kill me with words, Forget about me, it's what I deserve)
he's guilty for what he did (or is about to do, even),
the 'aching in my back' and 'stabbing pain' i think refer to guilt, or maybe his lack of backbone to do whatever should be done.
i think he's also saying that he never meant to hurt her. it's just that things got waaay too complicated.
towards the end of the song, you get the feeling that however sorry he is, he still doesn't love her, or even deserve her. he pretty much owns up to being a cold hearted bastard, saying that all he said were not sincere. "my words are as timed as the beating in my chest". i think that line says that his words are rehearsed, so i guess there's no sincerity or genuineness in them.
it's a sad take to it, but that's how i hear this song.
