It's about someone in a bad place. It think of it in general terms... the problem could be anything, but obviously he has changed for the worse, and is trying to go back to the way things were. He's very troubled and doesn't know how to go back, and sometimes isn't even sure he wants to, but he's trying.
It reminds me of my friend's struggle with her eating disorder though...
