from my interpretation i think its about a guy who was screwed over in a relationship hence(eyes open to splattered blood
cold tile mocks me as I arise to find the knife is still in my back) all of that is saying how she stabbed him in the back and now hes upset and pissed off and is trying to find a way to get back at her for what shes done to him.
and lastly he doesnt understand how she can just walk away from all this thinking nothing she did was wrong
