i think that could be about dependancy on alcohol, and the 'germs' are depressing parts of life, and how the alcohol has corrupted and taken over even the innocent parts of life, such as the milk.
also, the 'to kill all the germs' part could be an obvious excuse/denial/justification/whatever the word is of the alcoholism. also that he feels surrounded by 'germs', or negativity.
