I think this is the case with a LOT of Dir en grey songs.. the lyrics are often very disturbing, but it does really happen. Things like incest, rape, suicide and the like.. They do draw attention to very big taboos, although I don't think most people look at it that way. Like your explanation of the song, people think it's just fun, or interesting, or simply disgusting and don't even want to think about it (Freud would be so proud).
I don't know what Dir en grey and Kyo in particular want to accomplish with their lyrics but that's pretty much the impression I get: drawing attention to some serious problems in a nonconventional way
