She had known him for ages but never got together because they were always with different people, but just before she moved to America (to become The Veronicas) they FINALLY got together.
I remember reading interviews when they were together saying that they'd always call each other and stuff and how hard it was keeping a long distance relationship.
And i guess Jess was the one who broke it off and is telling Steve that know matter what happens she still loves him.
I was reading a interview and apparently when Jess was recording this she kept crying, so they did it like 20 times just to get it right. Then she sent it to Steve (who was ignoring her at the time), he heard it and loved it.
