Love the song...as for the meaning, I think he's really pissed off at this girl. Sort of obvious judging by lines such as "I don't give a damn about you, this much is true" and "Keep your advice, leave me alone."
I don't think it's to a girl; it's more to, you know, everybody. All those people telling him what he should do to improve his life, improve his health, etc. I think it's pretty clear since he says things like "don't like people who seem nice, talking in my head like a family of lice" and "I want this whole town to leave me alone." He wants to be left in peace to ruin his life or make bad decisions or whatever he likes without people nagging him about it all the time.
