I think that he and the girl he's singing to were once a couple with each other but now she's with someone else. He misses her and wishes that she feels the same way. For instance, my favorite line, "I just want to be near you when the things we've done come clear to you." He feels like she has forgotten everything that they've done together, all the good things while they were together.
Also, the part where he says "Making you a promise that I'll go out swinging while I'm out singing this song for us." I guess they both decided on the separation.. and he told her that he would try to move on, but really he's missing her and still wants to be with her.
