While this could quite possibly be about actual substance abuse, I tend to think it follows the trend of most of the album and is about a toxic relationship. I have some experience with this particular topic and also if anyone watched the later seasons of Angel, the relationship Wesley had with that evil lawyer chick is a rough example of what i am about to explain...

Its a relationship where there is no real love, but more of an addiction to each other and like a drug, you begin to lose yourself to it, you neglect your family and friends and you find your life is no longer recognizable. Everything seems to be like you are living in a semi-surreal dream that is quickly turning into a nightmare where you are drowning in this crazy mixed up world like a rip tide that keeps carrying you farther off the beaten path. Eventually you find that you have gone so far over the edge that you doubt you could ever go back to the way things were, but you lie to yourself, telling yourself that you can stop any time and everything will be fine . You try to break free of the person but you can't help yourself, you just keep going back for more. In a relationship like this you find yourself doing things you wouldn't normally do, which actually can lead to real drug use if you aren't careful.

This description is of something that happened to me so the song feels for me like a retelling of what i went through in the winter of 2000-2001
