I think it's about a disaster (a flood - maybe Katrina...)
And the singer can hear people crying out from the other side of the street (it's so flooded it seems like a river now) and no-one can sleep because of what they've seen/heard.
Someone is telling them to "Prepare for the flood" and even though it's obviously too late for that they're gonna try and help people.
Anyway it's an awesome song.
