Um, I don't think it's about emos, just because of the meaning of the word "decadence" (according to Wikipedia, "a lack of moral and intellectual discipline").
I'm pretty sure when he says "Yes, they know that you've hurt yourself another time / don't they know that you're full of pain already," what he means is that everyone else realizes what you're doing to yourself, but you don't. You may feel the hurt you're causing yourself, but you don't know why until he reveals the cause (decadence) by asking, "Decadence isn't easy, is it?"
