That line to me signifies that it is plain to see that he isn't dead or dying, but that all he talks about is his aches and pains. It seems she feels it is just a game he plays to get sympathy. Either that or, since this could be the last event they have together, that he really is in pain and dying, but that is all he talks about. Instead of enjoying their time together, they are miserable.
