This is probably my all time favourite song.
As aussiegirl87 said, Delta wrote it during her battle with cancer, most likely for Mark who she was dating at the time :S
Still, it's such a beautiful song. I think it's very much about love, and being vulnerable and afraid (of dying, being separated from the one you love, etc) and just saying everything you feel before it's too late.
The album version has a lot more of a happy feel to it than the live ballad version though...
