I think its about just all people in the world lieing all the time to themselfs and to others.
"Slither to sea" - Snakes... kinnda a simbol of bad.
"speakin' in tongues" Never really saying what you should.
"Deft as a surgeon's knife" a simily making refrence to lieing being as smooth as a sugron (usualy highly skilled)
