i think that the mask stuff is deffinatly on track, but also that he has some sort of a deformity ( either physical or mental) and no matter how much therapy or times he washes it, its always going to be there. Sounds like a scar that someone gave him when he was younger, and the anger towards that person keeps getting stronger but the scar wont go away.
