Um.. Being an emo, bulemic girl (or so I would assume..?). She's thinking her life sucks (it makes no reference to any real reason why) so she cuts and starves herself (so she won't be like 'those pigs', and doesn't care if it's bad for her health).
Her parents think it's not serious.
The voice is telling her to disregard her parents and their negative attitude that seems to egg her on. Then it tells her to either cut deep (enough for some damage, possibly death?), or don't cut at all.. In the end, she decides to not give up on life.
That's my understanding of the song; I don't understand the stanza with "portrait of skin"..
