Personally, I think this is actually about a guy who's actually GOING OUT with a girl, but realises that she doesn't really like him in that way, despite the fact he's really fallen hard for her.

But in the chorus he's saying that he'll be there for her until she tells him how he feels, and even after that he'll still be there, even though it'll kill him to see her with another guy.
