At first I thought he was addicted to someone in the past like an old ex-girlfriend. But after listening to the song a few times I realized it was more towards a drug addiction.
I secretly crave, crave that scent again
and now it's like he feels guilty that part of him still wants the drugs, cuz he thought he was done with it. but the fact that he still wants it makes him realize he needs to stay away from it. which is why he says

no i agree, i've never been Diagnost as a drug addict but i've felt the need to go get that drug even though i can't and he tells it like it is. plus the chick in the music video was like having horable withdrawl. most likely heroin
