I think it's got several meanings, but I mostly hear a story. A guy likes getting high, but for whatever reason, he can't. Maybe he got busted and he's on parole, who knows. Either way, he stages an accident (he puts fake blood in his hair and runs into a stop sign) so he can get high legally (on the downlow) on the very, very strong painkillers in the hospital. That's what it's about to me.
