I think the person has just lost someone that has spent some time in hospital - hence the line "eaten their LAST hospital meal". I agree with the other comments though that it seems to be about the injustice of someone that must've lead a pretty healthy life having died a horrible slow death wasting away. While those that choose to live unhealthily can still walk around, in an almost mocking fashion.
