I think this song does have to do with a relationship, with Andi and Cristina portraying the external and internal monologues we all carry on with ourselves. Whether this would be a healthy relationship or not depends on your perspective, but I think it has a lot to do with growing up and maturing and realizing your actions and words have a more significant meaning than we often put on them.
In the first verse, the lyrics could be talking about this person realizing their conduct has deeply wounded someone they care about, and no matter how much they try to hide, they can't suppress their feelings of shame and remorse for their behavior.
