I don't really get it either. I think it's about how you can get to rock bottom and nothing you do makes it any better ("you can take a lot... still come home looking thinner.")
The lines "will the lovely witch come down/kiss my pale, bleeding brow" make me really sad. I think they're about wanting to be loved/appreciated for your hard work and pain.
