I'm not sure its about falling in love cause "We've trying to reach you" unless its not an monogamous one.
I think it has to do with grief and how his girlfriend/partner is dealing with it. It may be a death that is particularly close to the girlfriend and has put her in a depressive spiral. Shutting herself off from everyone else and her partner and family.
hence the "Last night I dreamt I held you with me
Close enough to feel you breathe" and
"Last night I dreamt I held you with me
Closer still to see you leave" line for me have the feeling that his girlfriend isn't sleeping well hasn't been herself since the death and he's hoping the she comes back. hence the dreaming that she is close to him again.
Her family and b/f are trying to reach her, get through to her and help her. telling her that they are always there. "Just keep holding my hand" you know its ok "We all fall down like this sometimes"
and the last verse about walking in field gives me the imagery of funeral all the broken flowers and tears. its like she is dwelling stuck in an infinite loop and can't get past the grief.
