IMO this song is trent talking to himself. I think he's sort of left his body and is looking down at himself at a nin show. The people are like insects and from this trents point of view he looks just like any other person cept he's on stage, alone. well not alone but you know. He's saying how no one really understands him cos he can't put his thoughts into words suitable enough to portray what he's thinking but he acknowledges he has a second chance after his addictions etc ("I've become a million miles away") He then questions why so many people "love" him or admire him when he looks just like one of them. He gets all this love and such but still he's all alone. or at least lonely.
Anyway ye i think this song is basically him questioning his success and/or affect on his fans. just what i think though. lol
and i think with_teeth is a different album to other nin titles. it's not trent sellin out or w.e it's just a different album. sometimes i like it sometimes i dont depends on my mood, same with tds and phm.
