I think it's about something else... like the sick fuckers that were mean to him, the record company maybe, that made him a bit insane... so he feels bad about himself, but comes back for revenge on them by making THEM fucked up...

Now he feels as though not only is whoever trying to tear him down, but now everyone else is, too, in his mind.
So... that's kinda what i got out of the song. Weird, I know, but I don't really think it was about his ex... My Gift to You was for his ex... hm... it's just my opinion, though... I love this song anyway, whether it be about insanity or his ex-wife. Creepy and good.
