Yes. As the story goes along.. especially in the last song, you're lead to realize that there is no patient/psych having this talk. It's "Dean" talking to himself, doctoring himself.

I think this part is genius, because it's not just shown in the dialogue, but also enforced through the music. As you notice in the song, there are more split/layered voices (or at least more prevalant) than the rest of the cd, especially during the lines "the best storyline" ... when the contrasting tones mesh into one. Im pretty sure this was done on purpose, it supports the idea so well.

Then, at the end, it starts out with Ace (the psych) introducing himself as Dean (the patient) as the voices slowly transform from Ace's into Dean's.
