I guess there are three possibilities:
1.) the tattoo
2.) the tattoo as a symbol for a rape
3.) the tattoo as a symbol for first sex
Since the girl wanted to have the tattoo, I think that 3.) is the most likely. Maybe she wanted to make experiences/feel love, no matter "what it costs" - but this first experience marks her for the rest of her life and hurts her deep. (that's why she appears so lonely at the end)
