I think its that he knows someone has problems. Maybe its not even that she overdoses, or that she hurts herself. It seems to obvious that way.
I think that he knows somethings wrong, and he can't really understand it, but he's trying. He wants to understand, and he wants to be able to help ("put me on a plane and fly me to anywhere with you"). He wants to know what and why and how so that he can try to fix her, or at least console her the right way.
This song made me cry the first time I heard it. I got extremely sick right around the time it came out and no one could understand, no matter how hard they tried. And this song just made me feel...not so alone.
