I think the "it" refers to the issues in his head, or the "demon" as he called it himself sometimes. I could be wrong, it may be just my own interpretation as being an OCD sufferer myself. But to me it always made perfect sense that this was about someone who promised to love him and help him no matter what, whereas the person forgot about her promise and leaves him behind with the issues still there.
