This a really beautiful song, and it reminds me greatyly of Elliott Smith.
I don't know what this song is about, really, but I would venture to say someone is so distraught over something that they want to hurt themselves.
"You're sitting in your chair/no ones around/your lights are turned down low/and your skin feels about to tear/and you is no sound" is where the person is feeling despair and the singer is scared for them because they don't talk about how they're feeling ,"and all I am is frightened for you now/'cause you run and"
The singer wants to show the person all the good things about their life and though they are having a hard time now, they want to tell them how many good things are still to come, but the other person focuses on the bad "how can I show you that you're done/you're on your knees/picking up the pieces from your past/but there's nothing more to gather/holding on to moments that won't last/or would you rather end it all/do you wanna?"
I think this next part is about the other person having a son, "sitting there is him/he has your eyes/calling you to somehow join them and you're writhin' in your skin and buyin' his lies/not realizing that you're home/won't you come home and" I think that this son has died and that is why the person is grieving, they want to join the son in heaven.
The next bit is unclear to me and doesn't fit in to what I think the song is about, per se, unless it's the person calling like a suicide hotlin trying to get past the feelings, "no one believes you smile alone./you wanna retrieve your high on the phone/but when you hang up all I wanna do is help you to..."
Then it finished again with wanting to help the person over this hard time in their life by reminding them "Would you rather end it all... Do you wanna?" to show them that life goes on, and it does get better.
