I think the song is about the point in everyone's life when they get very old and realize they're close to death; they then think about how they lived their lives and question what they believe about the afterlife.
The following is nothing but my own opinion on each line's meaning. Keep the word OPINION in mind.
"I still feel the cold of long past days" = He realizes how much of his life is over, and how little time he has left. Realizing the grim, inevitable situation, he's overcome with a cold feeling.
"i knew my worth; put in my place" = He realizes his powerlessness under Death and realizes he must accept it.
"it's no surprise, I realized, some time before" = Now going back and thinking it over, he realizes that he should've seen it coming. He had thought about it before, but it never really hit him until now, as close as he is to death.
"December sun shines through haze" = Now we know the time setting...it's December. The sun is Death coming for him; the sun is as inavoidable as Death. The haze is his cloudy, unsure thoughts about Death itself...what to expect from it.
"i put my thoughts toward future days" = He switches his train of thought to the actual days he has left...how much time he may have, and how he will spend that time.
"it's no surprise; I close my eyes, and close the door" = He closes his eyes for the last time, and as such, has closed the door of life; there's no turning back now.
"Feeling so old, years pass like days." = As he closes his eyes, he realizes the situation he's in to the fullest and feels very old. As his life flashes before his eyes, the years pass as if they were only days. He realizes just how short life really is.
"Fastly changing, so many ways; my eyes perceive" = Watching is life flash before his eyes, he sees himself changing physically and emotionally throughout the years.
"Yes, I believe in nothing more" = He gives up any preconceptions of the afterlife that he's ever had and just takes Death as it comes, allowing whatever happens to happen...and he dies.
