Chino wants this girl so bad that he's trying to convince himself that no matter what, he'll get her ("And someday, I'll be with her, I'll be with you, I will") when deep down, he knows he's "sick", as in delusional, when he says this. One more note, the "first star" to be seen in the night sky is the planet Venus, which was the goddess of beauty in Roman mythology. Thus, it might not be his first love, but instead just a reference to how incredibly beautiful the girl is.... just a thought.
