I think this is about loosing a girlfriend to drugs.
"the way it killed to think that I had you first.. I had you first" --
sounds like he is saying that he had his girl first, and now she is giving up everything, including him for the drugs.
"I look into your eyes, see the chemicals, you're the whitest white; you've become a ghost. You're dead to me so why can't I let you go?" --
He looks in her eyes and can see what the drugs have done to her, and how they've changed her, and he wants to let her go because to him, she's already on the road to death.
"I had a love affair with distance" --
could mean that she is still here with him physically, but her mind is gone, her mind is with the drugs and it's in the distance.
Just my opinion. I also agree that it could be about a girlfriend thats moved away or something... but I agree with the drugs theory, personally.
