When I first heard this song, I thought that it was strange that Against Me was singing about something as simple as girls, since their songs are usually not so shallow. Looking at the lyrics more, and it may be a stretch, but it sounds like it's about a guy who has a disease like HIV (or another STD) and can't develop relationships with women because he'll be forced to tell them what's wrong with him.

As things start to get more serious he realizes that he must tell her the truth about his condition and why they can’t hook up.

His mistake is unprotected sex (with multiple partners... hmmm...)

Self-explanatory, the guy just wants to be healthy and the disease has left him unable to even get a bit of company from a girl.

The next part says that he has to accept what has happened, and I don’t know what the last part is talking about.

I’d hate to start rumors, but hopefully this song and the song “Searching for a Former Clarity” are just Tom’s overactive imagination, or maybe contain some other meanings that have gone over everyone’s heads.
