It could be about domestic violence on a surface level, but I get the feeling that they're aiming a little deeper. I think of it being written about a judgmental person, as a judgmental person would get off to pointing out when other people "blackout," and even "getting off" to it. And that maybe the very pointing out of other people's shortcomings would be what they mean by "punching skulls on the bathroom floor." I would think that the 2nd person to 3rd person switch has to do with the lyricist's moving from pointing out the general skull punching habits of individual this song is written about or to, to a specific instance in which the judgmental person beats on the "little girl" they sing to at the end of the song. I saw them play this live, and it was the awesome.
