I have to agree with W-T-F on most accounts but I'm going to add my opinion on key lines in it.
'I see the walls are closing in, and I feel I am in danger
My enemy is not alone, in the mirror there's a stranger
As I try to break out, my panic led to fear
Hopeless, I am alone, I'm afraid the end is near'
This means that it is getting too tough for him now and he is starting to not even recognize himslef when he sees his reflection because he was once someone that could accomplish it but now can't.
The 'panic lead to fear' part means that in his panic of not being able to accomplish this thing he is growing afraid. The last line is a key one because this means that he now feels afraid and alone and thinks that the proverbial end of his life has come.
NEXT KEY PART
'The truth doesn't always have to hurt'
Pure and simple, This is saying that it doesn't hurt to accept the truth which is not nesserceraly painful.
