I am not sure about what this song means because my english is not so good. I do not know what the expression "made of rats" does mean but I think this song is about an alcohol addicted bum with no "future plans" full of desperation and without any future, getting slowly crazy - shizophrenic. He starts to "talk with the back of" his "hand" and in the end of the song he starts to talk in plural "we" about himself.
