The whole relationship was a nice dream, and around the bend was an elevator he stepped into, which that turned out to be missing, and he fell down the shaft. Probably was cheated on or the chick turned out to have some horrible personality lurking under the facade.

Now he's back and realizes what a sham it'd been, and the girl is playing it off like her life is wonderful and she's got everything...and is rubbing it in his face.

He wonders though, if she's got to rub it in his face, is she really happy at all? Does she -really- have what she wants? Or is she only trying to make herself feel better about the fact she's actually empty inside?

I love the way he uses the spoiled little brother line to describe it, because child siblings are probably the most extreme case of people who like to tease with what they have when someone else doesn't have it. The thing is, most of the time, kids don't even enjoy what they're teasing people with all that much, and the teasing is usually the most fun they get out of whatever it is.
