I think its about how people and things around him are making him into a bad person (Leave your mark under my skin/Oh my how strong you are), and how he tries to not let the things around him control him (I will never belong to you), and how that is judging him unfairly (And if I decide that I am alive/Then I'm diseased and ungrateful/And if I confide that I am a liar/Then I'm diseased and ungrateful)
