I thought this site was for song meanings, not arguments over whih DT album is the finest. This album holds up very well to the test of time, sounding only a little dated in places.
I initially thought this song was about a guy trying to let a girl down easy when he dumps her, with 'dying' referring to the death of their love. However, on closer inspection this song a appears to be about a terminally ill family members wanting to commit suicide.

It's a heartbreaking song.

@bananaface You're right that it's about a terminally ill family member. Said family member turned out to be guitarist and song writer John Petrucci's father, who at the time was diagnosed with cancer.
