I think it means more than that... this is what it means to me:

They have a good relationship (whatever it might be) that's why she can tell him everything. He is waiting for her to say that she feels the same way. But she is trying to be careful not to hurt him. Because she doesn't and he knows it. That's why everytime she is cautions he knows and it sucks.
