I think this song is about two people who shouldn't be together but keep trying.

First, he describes them being together as an "awkward act." He also says "the chance to make it last has come and gone," so they screwed things up in the past and there isn't a chance that they can make everything like it was.

To me "Glass shatters with an unsteady grip No chance to catch the blood as it comes rushing in Too quickly pumping out from the inside Dripping into the patterns strewn across my thigh" means that something damaged their relationship (their relationship being the glass). Once damaged it caused injury to both of them (cutting of an arm) and they were both kept hurting each other, drawing blood. The blood that keeps flowing out and can't be stopped resembles this.

Two years after that the damage is done to their relationship she thinks there is still something there; maybe they never officially ended things or did so in a dubious way. He doesn't want to be with her but he has trouble telling her. Even though she doesn't think its over I think she's not REALLY in it like you would think. He describes her lips as "passionless" - not exactly a way to describe someone who is entirely into a relationship.

Finally, now that it is over he's glad to be free from trying over and over again and being trapped into things with her.
