To me this is just a really intimate song about his life. He's getting older, and looking back on past relationships - realizing that for the most part he's been alone and probably always will be kind of a loner. There's been good times and bad, but he has so many memories and true friends that will always be with him, and when those things aren't enough and he still feels depressed, he has the one thing that makes him truly happy - his music - to keep him going, and he wouldn't trade that for anything in the world.
