if anything they're disparaging self-harm references.
i could understand someone interpreting the imagery as drug-induced hallucinations. but i think there is more of a hint of schizophrenia here (hence 'catatonic')
'mary' is the doppelganger (think really hard about who this might be) she can't get away from, who's under her skin, and the toilet/fucked/eyes thing is a way of making her sound hideous, and expressing horror and disgust.
